POISE FOUNDATION POST SECONDARY SCHOLARSHIP APPLICATION INSTRUCTIONS

To apply for a scholarship, the student must include the following:

1. A completed application form. (click here for PDF Format Application)

2. Most recent academic transcripts.

3. A brief biography outlining educational history, community service, academic achievements and activities the student has been or is currently involved in. Applicants should also include any additional information, if any, they feel may better their chances of receiving a scholarship.

4. Please attach at least one of the following items as proof of financial need:

• Copy of FAFSA (Free Application for Federal Student Aid) or Financial Aid Award Letter
• Copy of most recent tax return (1040, 1040a, 1040ez, etc.) – include all pages and schedules
• Verification of income from social security, welfare, unemployment, etc.

                                                   

5. Attach an essay (300 words or less) on how you will use your education to give back to the community.
